You will be paired with a skilled and compassionate care worker from our team who understands your unique circumstances.

We figure out a plan

Step 2

Your care worker will work with you to create a plan that addresses your specific needs. They will discuss with you what services and organizations from the Connections of Illinois' Network could be of help and how they can assist you.

We get you connected

Step 3

You will be provided all the necessary information on how to receive aid or find resources. Your care worker will work with you to build relationships in the community, make appointments, and schedule meetings within our network.

We are here when you need us

Step 4

Connections is about community and we want you to know that we're in this together. Connections will help you obtain the resources you need to move toward self-sufficiency.
